Our case management team is here to support you in finding a therapist in the community for longer-term care. They can help you sort through local providers and your insurance to help you find a therapist that is right for you.

If you would like help getting connected with long-term care, complete the "Case Management Paperwork" at capsintake.byu.edu and then call 801-422-3035 to setup an appointment with one of our case managers.
